Many municipal licenses are processed and issued through the city clerk's office.  Licenses are required for the following operations or activities.

Tennessen Warning Notice

The government must give notice when collecting private or confidential information. 

Consent for the Release of Information

This form must be completed by each individual applying for a permit or license requiring a background check. 

Workers Compensation Compliance Certificate

MN Stat 176.182 requires evidence of compliance with Worker's Compensation Insurance coverage

Minnesota Tax Clearance Certificate

The City of St. Paul Park is required to provide the MN Commissioner of Revenue your business tax identification number and/or Social Security Number for each licensed applicant.
